The images that Amuscaria had for her avatar had Sentient Mushrooms, and they look cool, but I want to find out where their original sources came from?

Amuscaria is an artist and probably drew them.

BTW, will the Mushrooms have Faces, Arms, and Legs?

I don't understand your question.
Amuscaria's avatar mushroom clearly has a face and arms. The Easter egg/secret mushroom pickups in Hell-Forged also have faces and arms. No legs, though.
