Licencing?

Since I'm releasing most of my source code when I reopen my site. I need to decide on a licence. I'm not up on the whole world of licences. Here is what I want:

• If you make a branch project:
  Crlarify: If you take the whole program and just change a few things.
  • Let me know. Be it on the forum or just on the app's comment page.
  • It must stay open-source.
  • If my version of the project was free, yours must be too.
  • Keep a link to my site in the docs somewhere and preferably on the "about" dialog.
  • Please please donate!
• If you use a function:
  Clarify: If you want one of my functions for some reason.
  • Go right ahead
  • Leave my name in the comment header (if there is one).

I would read around various sites but I don't understand legalese and I was hoping someone could let me know if there is any kind of pre-done licence for what I want. If there isin't, I'm sure I could put a "licence.txt" file in the src package.
